perelman has won the first $1,000,000 clay millennium prize for his proof of the poincaré conjecture:

in 2000 claymath.org offered $7,000,000 for the solution to 7 problems ($1,000,000 each), namely,

birch and swinnerton-dyer conjecture
hodge conjecture
navier-stokes equations
p vs np
poincaré conjecture
riemann hypothesis
yang-mills theory

of which the poincaré conjecture was solved in 2002-2003 by perelman.

so far that is the only one which has been solved and the announcement on march 18, 2010 of the award to perelman is the only time the prize has ever been offered to anyone.

but will he accept it?

he also won a fields medal for it in 2006, but didn't accept it.

he has been informed of the award of the clay prize, but so far has not indicated whether or not he will accept it.
