a pathologist (the type who does laboratory testing eg blood) is their title "Dr" or not?

I think it would only be Dr if they had completed their PhD or a medicine degree....

Pathologists are doctors so yes, they have a Dr. in front of their name.

This is an oldish topic, but I thought that I'd clarify it anyway.

Whether a pathologist/lab tech has the title Dr. or not depends on their qualifications:

MBBS - Standard Medicine degree. True "Pathologists" have this qualification, and allows them to use the title Dr.

BMedSci - Bachelor of Medical Science. While not a "Pathologist" in the professional sense, people with this qualification generally work as as lab techs in and around pathology laboratories, carrying blood tests etc. Due to the nature of the degree, however, they are NOT titled Dr.

PhD - Not as common as the above two, and can be gained by pretty much anyone in any field these days. If someone has this, they can use the title Dr. This also applies to 'higher' doctoral degress, such as the DSc and DLitt.
