not really cause they would have gotten 1st internal, 1st external so it wouldn't affect the guy who came 2nd much, if at all.

The gaps between ranks are accounted for. So if 2nd is very close to 1st internally, this will show in their results. Goes for the whole cohort.

If 2nd is close to 1st internally then 2nd will get an internal mark close to that of 1st. So if 1st gets 100 externally (and thus internally too), then 2nd will get an internal mark close to 100.

Nope they look at the gaps between internal ranks as well.
